How do I complete and submit the SWIS Suite license agreement?

Certified facilitators work with schools to complete the SWIS Suite license agreement. The SWIS Suite license agreement is available for download here. 

This document is the license agreement specifying all of the terms and conditions for a given account.

The signature page may be completed electronically. We will accept both electronic as well as printed signatures on the submitted form. Both the school-level or district-level administrator's signature as well as the certified facilitator's signature must be present for the form to be processed. 

When the form is complete, submit page 6 in any of the following ways to the PBISApps Account Management & Support Team:

1. Email the form as an attachment to: accounts@swis.org
2. Fax the form to: 541-346-2471
3. Mail the form to: 1235 University of Oregon, Eugene, OR, 97401 Attn: SWIS Account

Please allow 2 weeks for account activation. As the facilitator, you will be notified by email when the account has been activated.
